History
Industrial Safety Engineering Company is an independent office established in 2004. The purpose of establishing this company is research projects and access to knowledge related to oil and gas industries. During these years, a large number of research projects done successfully despite of many problems including lack of confidence to private companies.
ISEC started investigation about the industrial incidents in 2009 to determine the conditions and circumstances which led to the events and to identify the causes of accident.
ISEC has found from experience that the causes of accidents are divided into several main groups:
- Equipment failure
- Human carelessness
- Operation fault
- Environmental conditions such as weather

Considering that lack of knowledge and awareness in dangerous situation is always one of the complementary causes and completes the chain of events, so apply experiences in these situations could reduce the effects of an accident and prevent a tragedy.
All the lessons and experiences from incidents are valuable and should be shared with the other companies and personnel to improve the safety level in organizations.
ISEC tried to encourage the industrial companies to investigate about their accidents as one of the elements of PSM (Process Safety Management) and share with the others to prevent same ones in the future.
ISEC believe that release of these experiences does not diminish the credibility of the company suffered from the incident, but shows the courage, implementation of knowledge management and social responsibility of the company’s managers in preventing similar accidents.
