Gas Pipes Moving and Laying Out by Loader

Accident Occurred On: 11/09/2016

On November 9, 2014, a young and inexperienced driver of the loader was asked to laying out the 10-inch gas pipes stored across the excavated channel. During throwing a belt around the three pipes, he slipped on the pipes and loosed his balance and fell between them. Unfortunately he died under pressure from the weight of the pipes. ISEC investigated about the causes of incident and produced the safety video and animation.
